CANCELLATION POLICY
Appointments must be cancelled or reschedule 24 hours in advance to avoid a late cancellation fee.
Appointments cancelled with less than 24 hours notice will be subject to a cancellation fee equal to the cost of service.
Cancellation fees must be paid before or during the next scheduled appointment.
A credit card may be required for any future appointments to ensure adequate notice of cancellation for future appointments at Kaizen Health Group.
Kaizen Health Group reserves the right to determine which of the following conditions listed above may be applicable.
